MedPrax Financial Policy
MedPrax, PLLC is a self-pay practice. We want you to know the expected cost of your care before services are provided. Please review this policy and ask us about any charge you do not understand.
Visit fees and payment
Our current visit prices are listed on our website and provided during booking. A $50 deposit is required to reserve an appointment. The deposit is applied toward the cost of that visit; it is not an additional fee. The remaining balance is due at the time of service.
The visit fee covers the evaluation and services described for the appointment booked. Laboratory tests, imaging, medications, injections, procedures, and other additional services may have separate charges. We will discuss applicable MedPrax charges with you before providing an additional service. If your care needs change during the visit, we will explain any change in price before proceeding.
Payment for a visit does not guarantee a particular diagnosis, prescription, procedure, or treatment.
Cancellations and rescheduling
Please give us at least 24 hours’ notice if you need to cancel or reschedule. With at least 24 hours’ notice, your deposit may be applied to a rescheduled appointment or refunded upon request.
If you cancel or reschedule with less than 24 hours’ notice, or do not attend your appointment, your $50 deposit is forfeited. We may waive this policy when circumstances warrant. If MedPrax cancels your appointment, you may choose a full deposit refund or apply it to a new appointment.
Laboratory testing and outside services
Laboratory testing is charged separately unless we specifically tell you it is included in a quoted package price. We will tell you the price of any MedPrax lab package before ordering it.
Some tests are performed or billed by an outside laboratory or imaging facility. Depending on how an order is placed, that facility may bill you or your insurance directly. Its charges and insurance decisions are separate from MedPrax’s visit fee. Please tell us before testing if you want to discuss available billing options.
An in-office test may also carry a separate charge unless it is included in the price we quoted you.
Insurance and reimbursement
MedPrax does not bill insurance for office visits. You are responsible for paying MedPrax’s charges at the time of service, regardless of whether you later request reimbursement from your insurance company.
Upon request, we can provide an itemized receipt with the information available for you to submit to your insurer. Reimbursement depends on your specific plan. MedPrax cannot guarantee that an insurer will reimburse any visit, test, or service, or predict the amount it may pay.
Estimates and changes in care
We will provide a good faith estimate when required for patients who are uninsured or choosing not to use insurance. An estimate reflects the services reasonably expected at the time it is prepared. If your clinical needs change, we will discuss additional MedPrax charges before providing those services whenever possible.
Refunds and billing concerns
If you believe a charge is incorrect, please contact MedPrax so we can review it with you. Overpayments and charges made in error will be refunded to the original payment method when possible. Processing times may depend on your payment provider.
Services already provided are generally not refundable. This does not limit your ability to raise a concern about the quality of care or dispute a billing error.
Acknowledgment
By booking to reserve my appointment, I acknowledge that I have reviewed MedPrax’s financial policy. I understand the $50 booking deposit, when it may be forfeited, that my balance is due at the time of service, and that additional services may have separate charges. I understand that MedPrax does not guarantee insurance reimbursement.
